Referee Resources

It is the coach’s responsibility to be in possession of the Medical Release Forms at all practices and games. Referees are not required, nor recommended, to ask coaches to prove that they have players’ Medical Release Forms in their possession at games. Referees should not require coaches to produce the forms at matches as this would, in effect, put the referee in the position of assuming a responsibility that properly belongs with the coach.

All fields required by AYSO National are locked down as required, and automatically appear as required fields in Blue Sombrero. All fields that aren’t required by AYSO National are automatically set to Not Required. These questions can be set to Required if the admin chooses to do so.

Safety Policies, Forms, & Insurance

Emergency Treatment Authorization Form – authorizes the treatment and/or care of players in cases of medical emergency.

Coaches must have a copy of the Player Registration Forms or acceptable Emergency Authorization forms with them at all times during practices and games in the event of medical emergency. It is the Safety Director’s responsibility to check with local hospitals and clinics to determine whether AYSO’s authorization is acceptable. If AYSO’s Authorization is not acceptable, obtain a copy of the necessary form and distribute it to all the Region coaches. Parents of each player must fill out and sign the appropriate form and return it to the coach for their keeping.
